Lower blepharoplasty

Puffiness, dark circles, excess skin, wrinkles, fine lines, pigmentation and any other change in the appearance of the lower eyelids are an important and increasingly frequent reason for our patients.

The changes in the appearance of the eyelids and the aged, tired face with the appearance of bags or hollows are mainly due to age, genetic or environmental factors.

Puffiness is caused by a prolapse of orbital fat (the fat around the eye that separates it from the orbital bone frame) which is caused by aging of the eyelids with a loosening of the septum which represents a membrane separating the eye socket from the eyelid . Pockets can also be present in younger genetically favorable patients.

Dark circles can be due to a hollow by a collapse of the fat or to the effect of the shadow, reflection of the bags above more marked by the difference in volume. They can also be pigmented, blue due to the very thin skin and the reflection of the vascularization below, or brown due to a high concentration of melanin.

The goal of the intervention is to have young, natural and rested eyelids. This consists of removing the fatty pockets (associated or not with a skin excision) but sometimes we redistribute these pockets to spread them along the periorbital rim (lower eyelid). In other patients we inject adipocyte cells to fill the hollows and find a homogeneous and balanced eyelid.

These actions can be associated with a mid-facial lifting or a facial lipostructure (injecting adipocyte cells or fat into the face).

During surgery we can perform a TCA skin peel, laser resurfacing, PRP or other medical procedures for better aesthetic results.

The surgery is done under AL in more than 90% of cases. The approach may be invisible transconjunctival, passing through the posterior part of the eyelid. In other cases, we make an incision under the ciliary (which will be hidden by the eyelashes) and the scar will quickly disappear.

The transconjunctival incision (inside the lower eyelid) has advantages : the scar is hidden and invisible and the incision does not need stitches, with much faster healing.

Dr Ben Saïd is an eyelid specialist. It also supports patients who have been operated on elsewhere with poor aesthetic or functional outcome, such as lower eyelid retraction, round eye, patients with a bad scar or ectropion (the free edge of the eyelid is turned outside) and patients with lagophthalmos (poor eye closure).
